HealthSPORT provides an environment for people to pursue and experience their health in a community of all ages. At HealthSPORT, we are active in community development and the support of community events and organizations. We welcome you to fill out our donation request form and we will contact you within 48 hours.

All requests must be submitted via this form; phone and in-house solicitations are no longer accepted and will not be returned. Requests should be submitted at least two weeks in advance of an event to allow for review. All donations are made in the form of a HealthSPORT gift certificate or service, which may be used as auction items, raffle prizes or for other fundraising efforts.
